Tattoo Consent Form
The form below is used at every tattoo session at Frøya Ink Tattoo Ålesund. It documents that you have received information about risks, aftercare and your rights, and that you give consent of your own free will. Please read the entire text before signing. The form is kept confidentially by the studio for a minimum of 5 years.
1. Personal information
The client fills in full name, date of birth, address, postcode and city, phone and email. The information is used only to document the session and for follow-up if any health issues should arise.
Identification
The client must present valid ID: passport, photo bank card, driver's licence or national ID card. The artist verifies that the presented ID matches the stated name and age.

2. Age confirmation and sobriety
- I confirm that I am 18 years of age or older, or that I have written parental/guardian consent (attached).
- I confirm that I am sober and not under the influence of alcohol, narcotics or any other substances today.
- I confirm that I have not taken strong painkillers or blood-thinning medication within the last 24 hours without informing the artist.
3. Health and allergies
Please answer all questions below. If you are unsure, consult a doctor before proceeding with the tattoo. Tick Yes or No and add a comment where relevant.
- Do you have diabetes?
- Do you have heart or cardiovascular disease?

- Do you have haemophilia or any other bleeding disorder?

- Do you have HIV, hepatitis or any other blood-borne disease?

- Do you have known allergies to latex, metals (nickel) or pigments/dyes?
- Have you previously had a negative reaction to tattoo ink?
- Do you have any wounds, infections or inflammation at the tattoo site?
- Are you taking blood-thinning medication (e.g. warfarin, aspirin)?
- Are you taking immunosuppressants or corticosteroids?
- Are you currently undergoing cancer treatment (chemo- or radiotherapy)?
- Have you had an organ transplant?
- Are there any other health conditions the artist should be aware of?

4. Informed consent
I confirm that I have been informed about and understand the following:
- A tattoo is permanent and difficult and/or expensive to remove.
- The process carries a risk of pain, swelling, redness and temporarily tender skin.
- In rare cases, infection, allergic reaction or scarring may occur.
- Colours may fade over time and appear differently on different skin types.
- I have received and understood the aftercare instructions provided by the artist, and will follow them.
- The studio is not responsible for complications arising from inaccurate or incomplete information provided by me.
- I may stop the session at any time if I wish to do so.

6. Photography (optional)
The studio may wish to photograph the tattoo for use in marketing (social media, website, portfolio). Tick one option:
- I consent to photos of the tattoo being used for marketing purposes. My face will not be published without separate consent.
- I do NOT consent to photos of the tattoo being used for marketing.
Photo consent can be withdrawn at any time by emailing [email protected]. Images already published in printed material cannot be recalled, but will be removed from digital channels.
7. Right of withdrawal
A tattoo is a personal service performed on your body. Under the Norwegian Right of Withdrawal Act § 22(c) (angrerettloven), there is no right of withdrawal for goods or services made to the consumer's specifications or which have a clearly personal character. Once the tattoo itself has begun, the session cannot be withdrawn. You may, however, ask the artist to stop the session at any time, but already performed work and any agreed deposit will not be refunded.
8. Parental/guardian consent (16–17 years)
Tattoos on persons under 18 are only performed with written parental or guardian consent. The young person must be at least 16 years old, and one parent or guardian must attend the studio in person together with the client and present valid ID. Frøya Ink Tattoo Ålesund does not tattoo anyone under the age of 16, regardless of consent.
As parent or legal guardian, I consent to my child, who is under 18 years of age (but over 16), receiving a tattoo at Frøya Ink Tattoo Ålesund. I am informed of the risks involved, that infection or allergic reaction may occur, and how such complications are treated and managed. I am aware that it is essential to follow the aftercare instructions provided by the artist, both to prevent infection and to give the tattoo the best possible healing.

9. Declaration and signature
I declare in good faith that all information provided in this form is accurate and complete. I understand that deliberately providing false information may lead to health complications and releases the studio and artist from liability should such complications arise as a result. I confirm that I am proceeding with the tattoo of my own free will.
